Call First Transit at 1-877-725-0569.  Requests must be made at least two business days prior to the trip.

Give First Transit the patient's information, including their medical card recipient number.  Also give the date and time of the medical appointment and the name, address and phone number of the patient's medical provider.

Tell the First Transit Operator that the patient will be riding with us; the Operator will give you an Approval Number.
